On the links to the right are some forms that we would like you to complete on or by your first visit. It will help us to better care for your child, so please complete the form in its entirety to the best of your ability. You can complete these and fax them to us prior to the visit or bring them in on the day of your visit. Our fax number is 561-793-2510.

Vaccine Policy Statement

We believe in the effectiveness of vaccines to prevent illness and to save lives. We believe that all children and young adults should receive all of the recommended vaccines according to the schedule published by the Center for Disease Control and the American Academy of Pediatrics.

Based on all available literature, evidence, and current studies, we believe in the safety of our vaccines. Research has proven that vaccines do not cause autism or other developmental disabilities. The recommended vaccines and their schedule are the result of years of scientific study and data gathering on millions of children by thousands of our brightest scientists and physicians.

Should you have doubts, please discuss them with your provider at your visit. Please be advised that delaying or “breaking up” vaccines goes against expert recommendations and can put your child at risk for serious illness. Finally, if you should choose not to vaccinate your child we will ask you to find another physician who shares your philosophy of medicine.

After Hours Policy

After Hours on call phone. One of the providers will be available 24/7 for emergency situations. The number will be provided on the main line if we are closed. Leave a message with the patient’s name, Date of birth, and the reason for the call, we will get back to you within 30 minutes.

*Please do not text unless asked to do so by the provider (i.e. pictures).
